Anthem has the highest rate of coverage denials overturned by the state
California’s independent medical review process allows patients under state-regulated plans to appeal denials. Among the largest health insurers regulated by the Department of Managed Health Care, Anthem was the most likely to have its coverage denials for all forms of health care overturned.
