Growth in Median Institutional Rates and Plan Premiums for Aetna Exchange Networks by State, 2024 - 2026

January 2024 - January 2026 Transparency in Coverage (TiC) data sourced and enriched by Serif Health. Posted negotiated rates were compared it to 2025 Medicare benchmarks derived from the national IPPS, OPPS, and ASC fee schedules for providers eligible to bill the code. Standard Serif quality filters were applied, excluding percentage-of-charge and per diem rates. Outliers below 50% or above 1000% of Medicare were dropped. Cumulative premium (rate) changes over the past 12 months come from CMS’s Unified Rate Review (URR) public use files for renewing Silver Exchange products reported by Aetna companies.
