This shows the facilities with the most recorded deficiencies in the latest inspection cycles, in this case between November 2016 and February 2020, as reported by the Centers for Medicare and Medicaid Services. The deficiencies cover a range of possible infractions: infection control, staff levels, food services, pharmacy services, abuse prevention and more. If a facility is bolded, federal records indicate that it has reported at least one case of COVID-19 among residents or staff.
Citations are recorded on a scale ranging from A to L, based on whether they are isolated or widespread, minimal or more severe. Deficiencies that score closer to A are deemed less serious, while those closer to L are most serious. Click here for more information on what each letter means.
