-Centene acquired WellCare in 2020. Metrics reflect performance prior to the merger.
-Arizona underwent a reprocurement and began implementing new contracts which merged physical and behavioral health care in October 2018. These rates reflect the plan performance before the new contract cycle, when they were responsible for only physical health services.
-In Arizona, Molina acquired Magellen in 2021. Rates reflect performance prior to acquisition.
-Measures from Georgia come from both the External Quality Review annual report and the accompanying validation report.
-Illinois underwent reprocurement in 2018. These metrics reflect performance prior to reprocurement. The state no longer contracts with NextLevel.
-In Kansas, all metrics reported come from an annual report to CMS, as opposed to the EQRO Annual Technical Report.
