Philly Hospitals With the Highest and Lowest Readmission Rates for Medicare Patients 65+

The two hospitals with the highest readmission rates, Jefferson Stratford Hospital, which includes Jefferson Cherry Hill and Washington Township hospitals, and Abington Memorial Hospital, are owned by Jefferson Health.

The 30-day readmission rate calculates the percentage of patients readmitted within 30 days of their initial discharge from a local hospital. The score includes patients who are readmitted to the same hospital, or another applicable acute care hospital, no matter the principal diagnosis, and are adjusted to account for hospital-level variations in patient condition and procedure. Speciality hospitals were excluded from the data. The data is taken from July 1, 2022 through June 30, 2023.

*Jefferson’s Stratford, Cherry Hill and Washington Township hospitals operate under a shared license held by Stratford.
