Negotiated Rate Percent of Medicare by Outpatient Service Line
December 2025 Aetna Open Access Managed Choice, BCBS Blue Card, Cigna National OAP, and United Healthcare Choice Plus Transparency in Coverage (TiC) data sourced and enriched by Serif Health. Posted negotiated rates were compared to 2025 Medicare benchmarks derived from the national OPPS, ASC, and PFS fee schedules for providers eligible to bill a given code. Standard Serif quality filters were applied, including exclusion of percent-of-charge and per diem rates. Code types of HCPCS and CPT were included, for all procedure codes with a corresponding Medicare benchmark.
