Healthcare Dive averaged NCQA data on insurers' commercial, Medicare and Medicaid plans. Only plans that reported data are included. Results for Elevance and Centene comprise the companies' major insurer subsidiaries (for Elevance, Anthem and Wellpoint; for Centene, Wellcare, Ambetter and Fidelis). Extraneous plans outside of these subsidiaries have been omitted. There is a small chance of plan ratings being double counted, or unrelated plan ratings sneaking in, due to the parameters of NCQA's search function.
