Drug Coverage and Restrictions for Medicaid
Data as of February 2025. For Medicaid, we calculated medication coverage by taking the number of drugs covered by each state plan as a proportion of those on the Medicaid “best plan”—defined as the total number of unique medications covered across all Medicaid plans. We then averaged these values, weighted by enrollment. The restriction rate reflects the enrollment-weighted average share of covered drugs requiring prior authorization, step therapy, and/or quantity limits.
