Note: Premium costs and income are measured at the family level. When premium payments exceed reported annual income, the share is coded as 100%. Families with zero income are excluded from the calculation. The lowest income quintile is divided into two categories: those with incomes below the Federal Poverty Level (FPL), and those with incomes above the FPL.
